Medical Release Waiver
We have a medical release waiver below that can be used three different ways:
1. Save it on your computer: You can complete the waiver, save it on your computer, and print it off to sign.
2. You can submit it: You can complete the waiver, click the Submit Form, and it emails us. You will still have to sign it before or the night of the event.
3. Blank waiver: You can also just print the waiver and complete all of the fields to turn in.
